What Happens in Gender-Affirming Voice Therapy? A Week-by-Week Guide to Your 5 Medicare-Funded Sessions

Voice is more than sound — it's identity, emotion, and connection. For many trans and gender-diverse people, aligning voice with gender can be deeply affirming. At Gender Voice Centre, we offer Medicare-covered gender-affirming voice therapy over five structured sessions, blending evidence-based practice with practical, supportive strategies.
If you're curious about what to expect, here's a week-by-week breakdown.
Week 1: Foundations & Forward Focus
We start by learning about you.
Your first session includes a detailed review of your intake forms — covering medical and social history, voice use, and reflux (using the Reflux Symptom Index). This context helps tailor the work to your unique needs.
Then, we introduce you to resonance, the quality of voice that influences where your sound vibrates. We begin with forward-focused hums — deceptively simple, but critical to get right. These hums form the foundation of the voice work ahead. If the placement is off, it can affect everything else — so we take our time here.
After your session, your speech pathologist sends you a personalised email or voice message demonstrating your home practice, so you can continue progressing between appointments.
Week 2: Straw Phonation, Tech & Mouth Shaping
In this session, things get playful — but always purposeful.
We introduce straw phonation by blowing bubbles into a glass of water. It might sound odd, but it’s a highly effective, evidence-based technique for balancing airflow and reducing vocal tension.
Next, we use the Voice Tools app to help you practise humming on different pitches and monitor your resonance.
We also introduce mouth shaping — learning to adjust the width and shape of your mouth to support your forward resonance. This can feel technical, so we often reduce the cognitive load with playful analogies (like imitating a race car!) to help you find the right placement without overthinking.
Week 3: Repetition, Feedback & Functional Phrases
Now we start reinforcing your new voice patterns.
Session 3 is all about high repetition and real-time feedback. We practise a wide range of resonant words and troubleshoot any sticking points.
You’ll also begin using your voice in functional phrases — everyday language like “yes please,” “no thanks,” or “just on card.” These high-frequency phrases are great practice for using your voice out in the real world with confidence.
Week 4: From Structure to Spontaneity
With a strong foundation in place, we begin moving beyond structured practice.
We support you in applying your voice to more spontaneous, natural settings. That might look like holding a casual conversation, answering the phone, or engaging in small talk. This session still includes feedback and guidance, but it’s about helping your voice feel integrated into real life — not just the therapy room.
Week 5: Expressive Voice & Independent Practice
By Week 5, you’ll have built a strong understanding of your voice and how to practise effectively.
This session focuses on emotional expression, projection, and fine-tuning. We help you refine the areas that still feel tricky and start using your voice more freely and expressively.
While five sessions won’t deliver a “finished” voice (and there’s no such thing as a perfect one), most people leave with a clear sense of how to continue practising and improving on their own.
What Happens After the 5 Sessions?
If you’d like to keep going, we offer private sessions, which are often covered by private health insurance. These can move to a fortnightly schedule or even monthly check-ins, depending on your goals and progress.
